TEAM REIMBURSEMENT FORM
Team requesting a reimbursement for a spring season game, must submit their request via the link below by June 30 following that season. Payment will NOT be rendered for anything submitted after June 30.
Team requesting a reimbursement for a fall season game, must submit their request via the link below by December 31 following that season. Payment will NOT be rendered for anything submitted after December 31.
All ID's for players deleted from a roster and or transfer to a new team must be turned into the league commissioner BEFORE they can be deleted from the roster.
INSURANCE CLAIM FORM
Please read the form carefully and follow all instructions. Once you have all the documents filled out email them to Patti Callahan, League Commissioner. Once the forms are received by Patti, she will then will sign the forms and generate the team roster. She will then send all the forms to NTSSA, who in turn will sign them and send them to USASA. This process takes several weeks. Eventually, the insurance company will contact you in regards to your claim.
DO NOT send any doctor bills!
