TEAM REIMBURSEMENT FORM

FALL SEASON
Team requesting a reimbursement for a fall season game, must submit their request via the link below by December 31 following that season.  Payment will NOT be rendered for anything submitted after December 31.

SPRING SEASON
Team requesting a reimbursement for a spring season game, must submit their request via the link below by June 30 following that season.  Payment will NOT be rendered for anything submitted after June 30.

SUMMER SEASON
Team requesting a reimbursement for a fall season game, must submit their request via the link below by August 31 following that season.  Payment will NOT be rendered for anything submitted after August 31.


INSURANCE CLAIM FORM

Provided information must be as complete and accurate as possible to ensure speedy processing. Once you have completed the form, it will be sent directly to USASA. The process takes several weeks. The insurance company will contact you in regards to your claim.

DO NOT send any doctor bills to NTWSA. They are not required at the time of submission but may be requested by the insurance company at a later date.
